ECEC Office Forms
This page is for business that involves current ECEC families. Here we will provide you with office forms that you can print out and fax (313-593-5505) or mail to the ECEC office (18501 Rotunda Dr., Dearborn, MI 48124).
EXAM WEEK CHILD CARE REQUEST
Submit this form to the ECEC Office by Friday, November 7, 2008 to request child care for your child during exam week and child care days (Dec. 10-19 & Jan. 5-9).

CREDIT/DEBIT CARD FORM
This form can be used to make a payment on your child's tuition. You may either mail or fax this form to the ECEC Office. Please be sure to include your signature and the V# on the back of your credit/debit card.
RETURNING CHILD(REN) APPLICATIONS
These applications must be accompanied by a deposit before they are processed. You may either mail the application with a check or credit card form or fax with the credit card form to the ECEC Office.
ANNUAL FORMS
These forms accompany the applications and should be updated once a year or as needed. You may either mail or fax these forms to the ECEC Office.
MISCELLANEOUS FORMS
- Financial Agreement - To be completed when your child enters the program.
- ECEC Petition - The petition process is used for approval of requests that are not within the ECEC's written policy. Petitions will then be reviewed by the Early Childhood Program Committee (ECPC). The petitioner will be notified of the ECPC's decision by the business manager.
